Preferred registration is online registration. However, if you do not want or are unable to use the online registration system, you may download the registration form (or download it for a friend), print it, fill it, and mail it to the District Office. You can still pay with a credit card on the form, or write a check.

Online (credit card)
After you register online, you will be able to pay online with a credit card. Payments will be processed through our secure online payment system, and will appear on your credit card statement as "Rotary5340.org".
5% registration cancellation fee. No cancellations after May 1, 2009
By Mail (credit card or check)
If you do not want to or cannot pay online, you may send your check or credit card information by mail to the District 5340 Office. You may still register online (just select the "Pay by Mail" option after you register), or you can send your registration information with your payment using the registration form above. Make checks payable to "Rotary District 5340". If you register online, send your registration number with your payment.
